to Executive Order 18, the following requirements must be met to allow
a registrar to issue a certified copy of a vital record: |

The person must prove they are related to the subject in one of the
following ways. They must be either the subject themselves, or
a. A parent, legal guardian, or legal
representative
b. Spouse (if of legal age)
c. Child (if of legal age)
d. Grandchild
(if of legal age)
e. Brother or Sister
(if of legal age)
f.
A State or Federal agency for official purposes
2.
The identity of the person requesting the document must be authenticated.
Acceptable forms of the identification are:
a.
A photo driver's license with address
or
b. Two forms of
ID with address from the following:
i. Non - photo driver's
license
ii. Vehicle registration
iii. Insurance card
iv. Voter registration
v. Passport
vi. Green card
vii. County ID
viii. School ID

6 HEALTH
VITAL STATISTICS PROGRAM |
Birth,
Death, Marriage Certificates and Marriage/Civil Union License Applications
are all part of Vital Statistics. Birth and Death Certificates
are valuable tools in assessing overall health, diseases and
causes of death. Birth Certificates can help health care providers
find out such things as the percentage of premature babies,
multiple births, birth defects, heights, and weights of babies
as well as any conditions appearing at birth.
Death Certificates are used to accumulate data on causes of
death relating to Age, Sex, Race, Diet and other conditions.
They are valuable tools in settling estates, ruling out suspicious
deaths and suicides and looking at how death rates and causes
of death vary according to City, State, and/or Country.
Marriage Certificates are used to establish legal name changes.
Usually the Bride takes the Groom's last name. For insurance
purposes a certificate is necessary for addition to the spouse's
insurance coverage. In estate settlement cases sometimes a marriage/civil union
license is needed to establish the relationship. |

7 WHEN
IS A BIRTH CERTIFICATE NECESSARY? |
| In
the State of New Jersey birth certificates count as 4 points
towards the 6 point renewal system to receive a license to drive
a vehicle and/or non driver identification. Note: a married
woman needs to bring an original raised seal marriage/civil union license
if using this form of ID. Birth certificates must be presented
when applying for a marriage/civil union license. Birth certificates are
necessary for a child to enroll in school and for anyone to
apply for and receive a passport to travel outside of the country.
*Some countries allow a raised seal birth certificate in place
of a passport. Birth Certificates are necessary to apply for
a Social Security Number which will enable you to work at a
legitimate business or company, for insurance purposes, retirement,
investment and pension funds as well as for general identification
purposes |

8 WHEN
IS A DEATH CERTIFICATE NECESSARY? |
| For
one, a death certificate is necessary to prove that a person
is in fact dead and that the death was by natural causes, suicide
or homicide. Insurance companies need a death certificate to
process claims for life insurance and Attorneys need a death
certificate in matters of estate settlement. When one wishes
to get married again and their spouse has died they need to
provide a copy of that spouse's death certificate in order to
acquire a new marriage/civil union license. Often, when the death was accidental
or intentional the certificate is necessary to prove matters
in a court of law. Genealogy or the study of one's ancestors
has created a new interest in acquiring death certificates of
deceased relatives. These death certificates help to discover
diseases that "run" in the family, the average age of death
of male and female relatives and even what occupations they
had. For example; it was not uncommon for men who worked at
the locomotive factory to die from diseases related to inhaling
chemicals and products associated with building locomotives.
Women and Children often died of "consumption" which is now
known as Tuberculosis. A cholera epidemic raced through Paterson
in the 1800's as well as an outbreak of Smallpox. These are
in fact some of the reasons the Paterson Health Department,
now the Paterson Division of Health, was established. |
